The Connected Sports Showcase 2023 Event Liability Release Form
WAIVER OF LIABILITY AND HOLD HARMLESS AGREEMENT
1. In consideration for receiving permission to participate in The Connected Sports Showcase 2023 (hereinafter referred to as the “Event”) to be held (Dates TBA), I hereby release, waive, discharge and covenant not to sue James Aguilar DBA Connected Sports Agency, Event directors, Athletic trainers, Stupak Community Center Las Vegas and each of their respective officers, directors, owners, other officials, employees, subsidiaries, affiliates and other related entities, servants, agents, representatives and successors (hereinafter referred to as “releasees”) from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or relating to any loss, damage or injury, including death, that may be sustained by me, or to any property belonging to me, whether caused by the negligence of the releasees, or otherwise, while participating in the Event, or while in transit to or from the premises, or in any place or places connected with the Event.
2. I am fully aware of risks and hazards connected with being on the premises and participating in the Event, and I am fully aware that there may be risks and hazards unknown to me connected with being on the premises and participating in the Event, and I hereby elect to voluntarily participate in the Event, to enter upon the above named premises and engage in activities knowing that conditions may be hazardous or may become hazardous or dangerous to me and my property. I voluntarily assume full responsibility for any risks or loss, property damage or personal injury, including death, that may be sustained by me, or any loss or damage to property owned by me, as a result of me being a participant in the Event, whether caused by the negligence of releasees or otherwise.
3. I further hereby agree to indemnify and hold harmless the releasees and each of releasees, from any loss, liability, damage or costs they may incur due to my participation in the Event, whether caused by the negligence of any or all of the releasees, or otherwise.
4. It is my express intent that this Release shall bind the members of my family and spouse (if applicable) if I am alive, and my heirs, assigns and personal representatives if I am deceased, and shall be deemed as a Release, Waiver, Discharge and Covenant Not to Sue the above named releasees.
COVID-19 LIABILITY RELEASE WAIVER AGREEMENT
In consideration of my participation in The Connected Sports Showcase 2023, I acknowledge and agree to the following:
Participation in the Event includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist.
I am following all CDC recommended guidelines as much as possible and limiting my exposure to COVID-19.
Following the pronouncements above I hereby declare the following: I am fully and personally responsible for my own safety and actions while and during my participation and I recognize that I may be at risk of contracting COVID-19. With full knowledge of the risks involved, I hereby release, waive, discharge the Organization, its board, officers, independent contractors, affiliates, employees, representatives, successors, and assigns from any and all liabilities, claims, demands, actions, and causes of action whatsoever, directly or indirectly arising out of or related to any loss, damage, injury, or death, that may be sustained by me related to COVID-19 while participating in any activity while in, on, or around the premises or while using the facilities that may lead to unintentional exposure or harm due to COVID-19. I agree to indemnify, defend, and hold harmless the Organization from and against any and all costs, expenses, damages, lawsuits, and/or liabilities or claims arising whether directly or indirectly from or related to any and all claims made by or against any of the released party due to injury, loss, or death from or related to COVID-19.
By submitting this form I acknowledge:
that I have read the foregoing Liability Release Waiver and understand its contents;
that I am at least eighteen (18) years old and fully competent to give my consent;
that I have been sufficiently informed of the risks involved and give my voluntary consent as my own free act and deed;
